The tracking inside the cuff of this elbow could have happened from a number of reasons.
1. Interfacial Pressure: The interfacial pressure is determined from the torque of the insulating plug to the elbow. If the end of the plug wasn’t torqued enough then there wasn’t enough pressure created which led to tracking.
2. cleanliness – If the inside of the elbow, or the insulating plug was not cleaned properly the pollution could have caused the tracking and the arc to take place along the interface.
3. Grease – Grease fills in all of the voids. If there wasn’t enough grease applied to the inside of the elbow or on the insulating plug interface.
4. Where the faraday cage meets the insulation inside the elbow. There is a slight step but too much of a step in this area could also lead to tracking.
